👁️ What is Stage 3 Keratitis?

Stage 3 keratitis is a severe, sight-threatening condition affecting the cornea (the clear front part of the eye).

It is most serious in:
👉 Neurotrophic Keratitis


🧠 What happens in the eye?

At this stage:

  • The cornea develops a persistent open wound (ulcer)
  • The deeper layer (stroma) begins to thin or “melt”
  • There is a high risk of perforation (a hole forming in the eye)

🚨 This is a medical emergency


📊 Stages of Neurotrophic Keratitis

⚠️ Why it is dangerous

The cornea becomes extremely thin, which can lead to:

  • Corneal perforation

🔄 How it progresses

  1. Starts as surface damage (Stage 1)
  2. Becomes a non-healing defect (Stage 2)
  3. Progresses to ulcer and tissue breakdown (Stage 3)

🩺 Management (Urgent Treatment)

💊 Medical treatments

  • Topical antibiotics → prevent or treat infection
  • Lubrication (artificial tears) → protect and hydrate the eye

👁️ Supportive treatment

  • Bandage contact lens (BCL)
    → protects the cornea and supports healing

🧬 Advanced treatment

  • Amniotic membrane transplantation (AMT)
    → helps repair and regenerate the cornea

🔧 Surgical options (if severe)

  • Corneal gluing → seals small perforations
  • Corneal transplant → replaces damaged cornea

⚠️ Differential Diagnosis

Must be distinguished from:

👉 Microbial Keratitis

  • Caused by infection
  • Often more painful
  • Requires antimicrobial treatment

  • Early treatment can save vision
  • ❗ Delayed treatment may lead to:
    • Permanent damage
    • Vision loss
    • Need for surgery
