What is the SPARK Tool?
The SPARK Tool is a survey used by our clinic to collect information about patients. This helps the clinic understand who you are and make healthcare fair for everyone.

  • Ability to pay for basic needs

  • Access to food and medication

  • Housing situation

  • Access to transport

  • Access to phone and internet

  • Access to utilities

  • Support from family or social groups

  • Employment status

Who can see this information?

  • Only your healthcare team can see your information

  • Your information is protected like all other health information

  • If used in research, it will be combined with others’ data and no one can identify you
